Adult League Complaints
For more information on Adult League Complaints:

This form collects your contact information, the type of complaint, details about the other person involved, a description of the issue, and allows you to upload supporting documents.
Match Make-Up
For more information on Match make-ups:
This form is used to reschedule matches delayed due to extreme weather. Captains must submit the Match Make-Up Form within 5 days of the original match, and the make-up must be completed within 14 days. Only lines that began (first serve attempted) are eligible for rescheduling under weather-related rules—defaults do not carry over. If bad weather impacts the make-up date, teams have 7 more days to complete the match, unless the season is ending, in which case DTA will assign a deadline. Captains should include their contact info, team details, original match date, and new date/time for each rescheduled line, if needed.
Grievance Form
Grievance Form:
The grievance form is used by the Dallas Tennis Association to formally report and address complaints or disputes arising from USTA league matches. Captains, players, or team members can complete this form to outline specific issues—such as match rule infractions, scoring discrepancies, unsportsmanlike behavior, or procedural violations—and submit it to league officials. It serves as the official record of the complaint, providing essential details like the event date, teams involved, description of the incident, and desired resolution. By standardizing grievance submissions, this form helps ensure that all concerns are addressed promptly, transparently, and fairly, reinforcing the integrity and sportsmanship of league play.
